Download Instructions

Where possible, each permit application form has been saved in two formats: in Adobe's Portable Document Format (PDF) or as a Microsoft Word Document.

If you choose to use a PDF form, and you don't already have Adobe Acrobat, you'll need to download the free Acrobat Reader exittdec3.gif (1060 bytes) software. Versions are available for Windows, DOS, Macintosh and various Unix platforms. The software is simple to use, with easy step-by-step instructions.  If you are using one of the latest browsers, and the latest version of Acrobat Reader, the file should open in your browser window.

The Word documents have not been compressed (zipped) and do not require special software.   They cannot be viewed using your browser, but must be viewed using your word processor.

Here's how to access the files:

  1. Click on the link of the file you wish to download.  A dialog box will pop up showing you where the file will be saved.  Make sure the directory you want is active, and make note of the file name so you can find it later.  Save the file and wait for it to completely download.
  2. To use the file, go to the directory where you saved the file using your file manager program.  Find the downloaded file.  Double click on the file name to open your word processor and view the file.

If you have difficulty with a document, contact the Department at [email protected], and we will make every effort to get you the requested document in a format that you can use.
